As the template itself links to, this is for disambiguation pages. It automatically adds "Category:Disambiguations".
Syntax

Usage
A disambiguation will be structured as such: The title of the disambiguation page is bolded, followed by the words, "may refer to:" Then a bullet list for the various things that have the same title. And finally, this template at the bottom of the page.
These pages must only be created when three or more things share a title. Otherwise use {{Redirect}} or {{Youmay}}.
